29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. On or about 05/05/2023, at the Shell Chemical facility located in Deer Park Texas, two Industrial Specialty Services' employees (ISS or Leak Sealer) were exposed to fire and chemical release hazards when the contract employees entered the Olefins 3 (OP3) unit without signing-in on the Unit Entry Log (UEL).
