29 CFR 1915.89(n)(3)(iv): The employer did not ensure that each lock and tag indicates the identity of the authorized employee applying it: A) Hull Shop on or about 4/13/2023, and or times prior to, the employer exposed employees to minor injuries in that the lockout padlock applied to the electrical disconnect of a Wells Horizontal band saw did not identify the authorized employee who put the padlock on.
