29 CFR 1910.25(b)(1):29 CFR 1910.25(b)(1): The employer did not ensure that handrails, stair rail systems, and guardrail systems are provided in accordance with � 1910.28. A) Vessel Laramie, on or about 4/11/2023, and or times prior to, the employer exposed employees to a fall hazard in that the stair rail system for the platform leading to the gangway did not have a top rail on the left open side going up and on a section of the right open side going up. B) Center of Machine Shop, on or about 4/13/2023, and or times prior to, a sets of job-made metal steps used to gain access to a "Plasmarc" cutting machine did not have guardrails or mid-rails on both open sides of the stairs. C) Hull Shop, on or about 4/13/2023, and or times prior to, the employer exposed employees to a fall hazard in that two sets of job-made metal steps used to gain access to a Burn table of the plasma cutter machine did not have guardrails or mid-rails on both open sides of the stairs. D) Vessel Khalid Bin Ali Lee, on or about 4/12/2023, and or times prior to, the employer exposed employees to a fall hazard in that the stair rail system for the platform leading to the gangway did not have a mid-rail on both sides.
