29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits before use: a) On or about 4/5/2023, Roof Area, Supports for piping associated with the NH3 system had been removed, piping not adequately supported. b) On or about 4/5/2023, Cellar W, A self closing valve was not provided with a terminus cap. c) On or about 4/5/2023, Cellar B, Unit B-2, a high pressure NH3 pipe was being used to support a low pressure NH3 pipe. d) On or about 4/5/2023, Cellar B, an "out of service" unit had a gauge indicating that the system was still pressurized. e) On or about 4/5/2023, Cellar K, Air Unit K1 was operating with vibration/ oscillation which was stressing pipe connections. f) On or about 4/5/2023, Cellar K, An accumulator (pressure vessel) had support metal welded to its end. Abatement documentation is required for these items.
