29 CFR 1926.1427(a): The employer did not ensure that each operator was trained, certified/ licensed, and evaluated in accordance with this section before operating any equipment covered under subpart C: On or about March 21, 2023, at 59 Mutiny Place, Key Largo, FL, employees were exposed to drowning, struck-by, and caught-between hazards while conducting pile driving operations with a crane operator that was not certified to operate the equipment.
