29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): On or about January 11, 2023, and thereafter, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to caught in and amputation hazards s in that the employer did not ensure that belt and pulley assemblies were adequately guarded. Instances of exposure include but are not limited to: Instance 1: Unguarded belt and pulley assembly located at Sawyers' clutch handle in main building. Instance 2: Unguarded belt and pulley assembly located at Off bearers' clutch handle in main building. Instance 3: Unguarded belt and pulley assembly located at Off bearers' edger in main building. Instance 4: Unguarded belt and pulley assembly located at Chop saw in main building.
