Text For Citation: 01 Item/Group: 010A Hazard:
29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: On or about January 11, 2023, and thereafter, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to caught between and crushing hazards in that the employer did not ensure that any powered industrial truck not in safe operating condition was taken out of service, and/or repairs were made by authorized personnel. Instances of exposure include but are not limited to: Instance 1. Komatsu Forklift: Anti-slip pads on the foot pedals were missing and damaged rear tires. Instance 2. Toyota Forklift: Horn button and anti-slip foot pads on the foot pedals were missing, damaged rear tires. Instance 3. Hyster Forklift: Front and rear damaged tires.
