29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: On or about January 11, 2023, and thereafter, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to caught between and crushing hazards in that the employer did not ensure that powered industrial trucks that employees were operating in the workplace had all labels in place on the operator control levers. Instances include but are not limited to: Instance 1: Komatsu powered industrial truck operator control levers. Instance 2: Toyota powered industrial truck operator control levers.
