29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about January 11, 2023, and thereafter, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to fall hazards related to tripping or stepping into or through floor holes. The employer did not ensure employees who were conducting green line and re-saw work were adequately protected from tripping or stepping into or through holes that were less than 4 feet (1.2 m) above a lower level.
