29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about January 11, 2023, and thereafter, employees engaged in sawmill activities at the worksite located at 33643 MO-6 Brashear, Missouri 63533 were exposed to fall hazards in that the employer did not ensure that ladders were inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. Employees were accessing an elevated platform utilizing a broken extension ladder to conduct re-saw and lumber stacking work.
