Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck by hazards while tailing pipe: a) Longitude 40.6349, Latitude -104.658, near Greeley CO: On or about December 21, 2022, and at times prior, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that rig crew employees were exposed to struck-by hazards. Employees working on Rig US-142 were exposed to struck-by hazards while moving pipe across the rig floor without the proper use of a tail rope. The employer did not ensure the adherence of safe work practices as outlined in their Standard Operating Procedures for tailing pipe. Feasible and acceptable abatement methods include, but are not limited to: 1) Ensure the enforcement of already established company standard operating procedures and safe work practices as described in the company training documents. 2) Ensure all employees who are tasked with tailing pipe have been trained on proper and correct usage of the tail rope.
