29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) 3155 Primary Profiling Module: On or about December 21, 2022, the employer exposed employees to crushed-by hazards in that the employer failed to ensure the lockout/tagout procedure for the equipment with revision date 4/7/2020 clearly identified all the specific steps to be followed by employees to isolate and dissipate all the hydraulic energy to prevent unexpected module movement when employees were engaged in servicing and maintenance activities.
