29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval. On or about December 15, 2022, and at times prior thereto, employees used a job-made wooden trash box attached to a telescoping forklift without the written approval of the manufacturer, exposing employees to the hazard of being stuck by falling material or the trash box.
