29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Maintenance shop; on or about December 14, 2022, the employer exposed employees to crushed-by hazards in that employees were allowed to perform maintenance and servicing on equipment without being trained in lockout tag out.
