29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection are protected from being cut, abraded, melted, or otherwise damaged: a) Building 19/20, Roof - Employees used an 11-foot self retracting lifeline as part of a personal fall protection system and the webbing was not protected from being cut, abraded, or otherwise damaged while working next to the sharp edges of a hole cut into a corrugated metal roof, on or about November 2, 2022. b) Building 19/20, Roof - Employees used a 20-foot self-retracting lifelines as part of a personal fall protection system and the wire rope was not protected from being cut, abraded, or otherwise damaged while working next to the sharp edges of a hole cut into a corrugated metal roof, on or about November 2, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
