29 CFR 1910.140(c)(18): Personal fall protection systems were not inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components were not removed from service: a) Building 19/20, Roof - The employer did not sure that personal fall protection systems were inspected as required and/or defective components were not removed from service. An employee donned a full body harness and 6-foot single leg polyester shock absorbing lanyard as part of a personal fall protection system and the equipment was visibly damaged, deteriorated, and had excessive wear, on or about November 2, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
