29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) For the Rayco production area, on or about September 19, 2022, and at times prior to, the employer exposed employees to caught-in hazards, in that, cycle pause buttons and emergency stop buttons and devices were used to de-energize the nailing gantry component of the Rayco pallet assembly machine, model Pallet Pro, Serial # P201849, for pallet jam clearing. b) For the Rayco production area, on or about September 19, 2022 and at times prior to, the employer exposed employees to caught-in hazards, in that, cycle pause buttons and emergency stop buttons and devices were used to de-energize the nailing gantry component of the Rayco pallet assembly machine, model Pallet Pro, Serial # Serial # P201957-0PP, for pallet jam clearing.
