29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Worksite - On or about August 29, 2022, two employees were exposed to tipping and fall hazards when working on top of a scaffold (WEN Scaffolding, Model: 31110) stacked 3 high with each scaffold stack measuring approximately 65 inches in height and the wheel height measuring approximately 6 inches. Therefore, the two employees were working from a height of approximately 16 feet 8 inches. The two employees were both hospitalized from this incident. The employer reported the hospitalizations to OSHA on August 31, 2022.
