29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Worksite - On or about August 29, 2022, two employees were exposed to tipping and fall hazards when working on top of a scaffold (WEN Scaffolding, Model: 31110) stacked 3 high with each scaffold stack measuring approximately 65 inches in height and the wheel height measuring approximately 6 inches. The two employees were working from a height of approximately 16 feet 8 inches. The employer did not train these employees on scaffolding hazards.
