29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: a) Worksite - On or about August 29, 2022, two employees were exposed to movement, tipping and fall hazards when working on top of a scaffold (WEN Scaffolding, Model: 31110) stacked 3 high with each scaffold stack measuring approximately 65 inches in height and the wheel height measuring approximately 6 inches. The two employees were working from a height of approximately 16 feet 8 inches with two wheels in the locked position and two wheels in the unlocked position.
