29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: a) Worksite - On or about August 29, 2022, two employees were exposed to tipping and fall hazards when working on top of a scaffold (WEN Scaffolding, Model: 31110) stacked 3 high with each scaffold stack measuring approximately 65 inches in height and the wheel height measuring approximately 6 inches. The two employees were working from a height of approximately 16 feet 8 inches. The base width dimension on the scaffold was 28 inches. The height to base width ratio for the scaffold was approximately a 7:1 ratio. The scaffold was not restrained from tipping by guying, tying, bracing, or equivalent means.
