29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, the employer did not ensure that powered industrial truck operators completed training and evaluation prior to operating the vehicles to move slabs of granite and quartz. Forklift operator was observed using a boom attachment with a slab clamp to lift large slabs of granite and quartz throughout the facility. At the time of the observation the operator had not received training and/or evaluation on the use of the forklift and attachments thereby exposing employees to struck-by hazards.
