29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Signature Cabinetry located at 1285 Alum Creek Drive, COLUMBUS, OH 43209: On and preceding August 23, 2022, employees were exposed to machine hazards associated with moving parts during tooling changes and servicing on machinery with multiple energy sources. Employees perform blade changes and periodic maintenance on a GMM Extra CNC stone saw. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect and all other energy sources [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(S)(i)J, and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. The following condition exposed employees to caught-in and struck-by hazards.
