29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. The employer did not provide locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware as required. On or about July 28, 2022, and at times prior thereto, the employer did not provide energy isolating devices to employees who were performing maintenance on the Kebo Pin Setter Bowling Machine, exposing employees to caught-in hazards.
