29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces. (a) On or about September 1st, 2022, and at times prior thereto, safe access and egress was not provided to employees at the Basement #2 tunnel under 22 shop. A stair with three steps and risers, immediately abutting the tunnel walking working surface had steps that were narrower than 22", was not kept clean of debris, and was not provided with a handrail for employee use, exposing employees to slip, trip, and fall hazards.
