29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l). a) Jobsite located at 546 Valor Dr, Downington PA: An employee was operating a rough terrain forklift to hoist trusses near and around employees, without the successful completion of a combination of formal instruction, practical training and workplace evaluation prior to operating such equipment exposing employees to a struck-by hazard. Condition noted on or about July 21, 2022.
