29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. (a) ReyLenn Construction Company, Inc., 8355 E 32nd Ave., Denver, CO 80238, Solana Central Park Project: On or before July 12, 2022, and at times prior, ReyLenn Construction Company, Inc., as the controlling employer did not protect workers at the site from an exposed pipe hole 5 inches in diameter located in a walking, working surface just off of the northwest corner of the parking garage in the northwest corner of the project site. The hole was not marked, covered, or blocked off to prevent employees from tripping in or stepping into. Workers were exposed to trip and fall hazards.
