29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a. On July 5, 2022, in the maintenance shop, the employer did not conduct a periodic inspection of the "operational lock out tag out procedures" utilized when trying to identify and diagnose a hydraulic leak in the hopper of the garbage trucks while the trucks and blades were operational, exposing employees to a struck-by and crushing hazard.
