29 CFR 1926.502(i)(4): Fall protection systems criteria and practices. Covers. All covers shall be color coded or they shall be marked with the word "HOLE" or "COVER" to provide warning of the hazard. Note: This provision does not apply to cast iron manhole covers or steel grates used on streets or roadways. a) On or about June 21, 2022, the employer did not ensure that the covers used on holes were marked as "HOLE" or "COVER" to provide any warning of the hazard to the employees working in close proximity to the holes.
