All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. a) On or about June 21, 2022, the employer did not ensure that five holes 24" wide and 10 ft. deep that were recently drill on the jobsite were covered, exposing employees to the fall hazard of unprotected holes.
