29 CFR 1926.501(b)(4)(i): Fall Protection. Duty to have fall protection. Holes. Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) On June 21, 2022, The employer did not ensure that employees working around unprotected drilled holes were protected from falling by a fall protection system such as, personal fall arrest systems, covers, or guardrail systems erected around such holes. Open holes were measured to be 10ft. deep and were 24" wide.
