29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) At the fabrication shop located at 1138-1 Pondella Road Cape Coral, Florida: On or about June 17, 2022, the employer used a jib-boom attachment and a stone slab lifting clamp on the CLARK, Model: C25CL (LP) forklift, without requesting the manufacturer's prior written approval, exposing employees to crushed-by and struck-by hazards.
