29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about March 25, 2022, at TSL Terminals located at 5646 Kansas Avenue Kansas City, Kansas, a client site, employees of Gomez Roofing LLC were exposed to fall hazards. Employees engaged in roofing and gutter installation activities were exposed to unprotected skylights and fall hazards. An employee was fatally injured when he fell approximately 24 feet from the upper roof to the concrete ground below through an unprotected skylight. Fall protection systems, covers, or guardrail systems were not utilized to protect employees from falling through skylights.
