An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. At 1776 Cecos Lane, Arnold, Mo, 63010, On April 11, 2022 and at times prior thereto employees were exposed to struck by and caught between hazards associated with the unexpected energization of equipment due to a lack of orderly shutdown to avoid increased hazard to employees as a result of the equipment stoppage.
