29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 03/17/2022, in the formulation department, the employer did not ensure that locking and tagout devices were affixed to the energy sources of the AFMG-56 MKIII Automatic Feed Mixer (Meat) Grinder when employees were installing the rotating arms inside the grinder or conducting maintenance activities.
