29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Production shop: On or about March 1, 2022, and at times prior, CNC machine centers, including those manufactured by HAAS, Fadel and Toyoda, were operated with no key control procedures to prevent operators from switching between production and maintenance modes. Keys remained in machines during production which allowed operators to bypass interlocked access points while the machining center was energized and susceptible to unexpected, rapid movement. Additionally, all HAAS machine center programming and maintenance mode keys were interchangeable between modes and machines, meaning the presence of even one key allowed for switching between maintenance and operator modes on all HAAS machines. b) Production shop: On or about March 1, 2022, and at times prior, CNC machine center interlocks were ineffective due to missing parts with efforts taken to defeat the interlock function designed to prevent employee access while hazardous machine movement was occurring at the point of operation. c) Production shop: On or about March 1, 2022, and at times prior, workers were exposed to laceration and amputation hazards while operating the Hardinge lathe, Model HLV-EM without a guard to cover the work-holding device.
