Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about February 09, 2022, untrained employees were allowed to operate powered industrial vehicles (e.g.: the company's forklift) at the site without having received formal instruction, including operator evaluation to determine the retention of the information. While in the process of loading fiberglass street poles that measured 6 inches in diameter and tapered to 2 inches at the top and approximately 35 ft. in length, an employee attempting to aiding in the loading of the streetlight poles was injured when he was knocked off trailer. A second employee who was tasked with operating the fork truck, was in the process of unloading the poles as he backed-up he raised the forks causing the tiered bundle of poles roll. Both employees were untrained.
