1910.9(b): The employer did not train each affected employee in the manner required by the standard: (a) Lowes' Home Improvement, LLC, dba Lowe's of Rapid City (Store #1634) at 2550 North Haines Avenue, Rapid City, SD 57701: On or about and at times prior to February 3, 2022, the employer did not ensure a damaged mobile ladder stand, Lowe's ID #619054 located in the main floor carpet area, was removed from service or repaired by a qualified technician. This condition exposed employees to fall hazards greater than four feet due to potential structural failure on mobile ladder systems. (b)Lowes' Home Improvement, LLC, dba Lowe's of Rapid City (Store #1634) at 2550 North Haines Avenue, Rapid City, SD 57701: On or about and at times prior to February 3, 2022, the employer did not ensure a damaged mobile ladder stand, Lowe's ID #206339 located in the main floor motion light area, was removed from service or repaired by a qualified technician. This condition exposed employees to fall hazards greater than four feet due to potential structural failure on mobile ladder systems. (c) Lowes' Home Improvement, LLC, dba Lowe's of Rapid City (Store #1634) at 2550 North Haines Avenue, Rapid City, SD 57701: On or about and at times prior to February 3, 2022, the employer did not ensure a damaged mobile ladder stand, Cotterman Co. brand serial number 09 02 located in the main floor electrical hardware area, was removed from service or repaired by a qualified technician. This condition exposed employees to fall hazards greater than four feet due to potential structural failure on mobile ladder systems.
