29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On January 25, 2022; employees were exposed to falls of approximately 15 feet while installing metal covers over roof penetrations and holes on a flat roof without fall protection. Employees were working next to a skylight measuring 32 inches by 57 inches, an eight inch round spot penetration and a hole measuring 36 inches by 36 inches. Note* Abatement certification AND documentation are required for this item.
