29 CFR 1910.252(a)(2)(iv): The individual responsible for authorizing cutting or welding operations did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit: a) Worksite, 20 Marina Drive, Coeymans, New York, Maintenance Shop: On or about to January 20, 2022, an employee was welding a bracket on the side of a 275-gallon used oil tank that was approximately ? full of used oil and other unknown combustible liquids. The employer did not designate precautions to be followed in granting authorization to proceed preferably in the form of a written permit prior to conducting welding work.
