Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 20, 2021, at 12560 Sheets Road, Rittman, OH 44270, the employer failed to ensure employees adjusting the light sensor on the top sheeter utilized written Lockout/Tagout procedures. Employees only utilize the light curtain which does not shut power down to the clamps on the top sheeter and an employee had two finger tips amputated. Unilock Chicago, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1910.147(c)(4)(i) ), which was contained in OSHA inspection number 1249325, citation number 01, item number 005 and was affirmed as a final order on February 1, 2018, with respect to a workplace located at 301 E. Sullivan Rd., Aurora, IL 60505.
