29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred: Company: On, or about, December 16, 2021, an employee who was employed through a temporary staffing service who was under the direct supervisor of the employer suffered a finger amputation while unjamming a packaging machine. The injury was not entered on the employer's OSHA 300 Log of Work-Related Injuries and Illnesses within seven days of the occurrence.
