29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee. On or about December 1, 2021: Employees engaged in framing activities at the Runza construction site located at 8910 Fort St. Omaha, Nebraska, were exposed to fall hazards in that an employee was using a fall protection system that was used in such a way that the anchorage point being used by the employee could not support 5,000 pounds per employee. The fall protection system was used in such a way that the anchorage point (used wooden truss) moved allowing the employee to contact the ground resulting in injury.
