29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. 18820 Cortez Blvd., Brooksville, Florida, On or about December 13, 2021, employees were exposed to fall hazards of more than 20-feet while installing a new Thermoplastic Polyolefin (TPO) roof covering system over the existing Purlin Bearing Rib (PBR) metal panels and skylight type corrugated fiberglass panels. The roof had a 1:12 pitch; 20-feet to the eave and 25-feet to the center top. Fall protection equipment was not installed or utilized nor were covers placed over the fiberglass panels to prevent employees from falling.
