29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) On or about November 19, 2021 - 22319 AL 79, Scottsboro, AL, employees were exposed to fall hazards when components with defects such as, but not limited to, missing weight limit labels, broken rope threads, and missing manufacturer inspection criteria, were not inspected and removed from service.
