29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. A) 17 W Main St Bound Brook, NJ 08805 Holes approximately 6 inches by 6 inches were cut into the plywood floor to allow for water drainage from rain. The holes were left uncovered and created a slip, trip and fall hazard. Hazard noted on or about 11/10/21
