29 CFR 1910.212(a)(3)(iv)(i): The employer did not provide and ensure the point of operation guarding was provided: On or about November 04, 2021, and at times prior thereto, the employer did not ensure that the A3 forming machine was equipped with a point of operation guard to protect employees from moving parts while conducting manual temperature testing. Thus, exposing employees to amputation hazards while performing this task as the machine was still in operation.
