29 CFR 1910.134(a)(2): A respirator was not provided to each employee when necessary to protect the health of such employee: Harmony House - On or about October 1, 2021 and continuing afterwards, healthcare workers were exposed to COVID-19 from coworkers and while providing direct care to suspect or confirmed COVID-19 patients, while wearing inadequate respiratory protection, such as surgical masks. The appropriate respiratory protections are N-95 respirators. The employer did not require or 'ensure use' of N-95 respirators. Between October 1, 2021, and October 18, 2021, fourteen employees became infected with COVID-19 and lost between 10 and 56 days of work.
