On or about September 17, 2021, and October 19, 2021, laser operators entered the material handling area to clean the index table slats. b. On or about October 01, 2021, to October 08, 2021, laser operators entered the material handling area to replace index table slats. c. On or about September 17, 2021, laser operators entered the material handling area to perform cleaning activities.
