Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards during crane operations: a. Liberty Oilfield Services, Inc., 9540 E. 104th Ave Henderson, CO 80640: On or before July 28, 2021, the employer did not ensure that workers performing maintenance on top of a Hydraulic Horse Power Pump Truck (HHP) were protected from struck-by hazards from movement of the AmQuip 15 ton Bridge Crane in Bay 1. While moving the bridge from one end of the bay to the other, the crane operator was in a location which made it impossible for him to see the path of travel of the crane, and obstructions in the path of travel, creating a struck-by hazard to workers in the path. Abatement Note: Feasible and acceptable abatement methods include to position the crane operator so the path of travel of the crane, load, and any obstructions in the path of travel can be seen, or to use a signal person, per ASME B30.2-16 section 2-3.3.4.
